Oct 31, 2009 · The Blood Ties quest is a tricky challenge. You need to find Lucy West's brother, Ian, and give him a letter. This is complicated when the town of Arefu is attacked by The Family. You'll need to track down the location of The Family's hideout in Meresti Trainyard or travel through Northwest Seneca Station.

Feb 23, 2014 · Fallout 3/Blood Ties. In Megaton, Lucy West wants to deliver a letter to her family in Arefu, and asks you to deliver it for her. You can also start the quest if you reach Arefu and speak with Evan, or if you encouter The Family in their home and learn about the Arefu incident.
With a high enough medicine skill or with enough intelligence, you'll be able to convince Vance to protect (or just leave alone) Arefu in return for Blood Packs.
You'll gain the Hemotophage Perk which allows you to consume Blood Packs for 20 HP instead of 1 HP. You can also sell Blood Pack to Vance for 15 Caps a piece.
