Karen kills Hastings with a scalpel before returning home armed with his gun, where she kills Roselli, also a Satanic disciple, only to find Delia waiting for her, holding Alexander and drawing Karen's attention to the 666 symbol, clearly displayed on the palm of her brother's hand. Influenced by the baby's power, Karen ends up taking her own life.
The Antichrist, also known as the Beast or the Prince of Darkness, is the child of Satan (or, in the case of Damien Thorn II or Alexander York, the grandchild of Satan), and the archenemy of Jesus Christ. He is destined to rise to political power and cause war, corruption, and chaos on Earth during Armageddon, before being defeated by Christ.
In Omen IV: The Awakening, Damien is revealed to have a biological child in Delia, who was originally assumed to be the Antichrist reborn. According to the plot of the previous film, the Kingdom of Christ began much earlier and Armageddon was not prevented, but only delayed.
Realizing that the Antichrist is Alexander, Karen kills Hastings then heads home to try to kill Alex and Delia. After killing Lisa Roselli (Delia's new nanny and one of the Satanists protecting Alexander), Karen confronts Alex and Delia.
