In pathology and anatomy the penumbra is the area surrounding an ischemic event such as thrombotic or embolic stroke. Immediately following the event, blood flow and therefore oxygen transport is reduced locally, leading to hypoxia of the cells near the location of the original insult.

It is usually located around an infarct core which represents the tissue which has already infarcted or is going to infarct regardless of reperfusion.
The ischemic penumbra was defined as “the ischemic brain with the CBF between the upper threshold of electrical failure and the lower threshold of energy and ion pump failure” [ 2 ]. Soon after its introduction, ischemic penumbra has become the diagnostic and therapeutic target for ischemic stroke [ 4 ].
