The FDA has approved Valneva’s chikungunya virus vaccine Ixchiq for adults at risk of exposure to the virus. The company will focus on selling the first-in-class vaccine to US travellers heading to countries in the tropics and subtropics where the disease is endemic.
Chikungunya is a mosquito-borne viral disease that causes fever, joint pain, rash, headache and muscle pain. Some 5 million cases of infection have been reported over the past 15 years, mostly in the Americas, Africa and Southeast Asia. Infected travellers sometimes trigger local outbreaks in the southern USA and Europe, and the warming climate is increasing the range of the mosquitos that carry the disease.
Ixchiq, formerly VLA-1553, is a single-dose, live-attenuated vaccine. Previous chikungunya vaccines had failed, including live-attenuated vaccines that remained at risk of reversion to pathogenic virus. Whereas earlier candidates were attenuated by repeated cell passage, VLA1553 was attenuated by deleting the nsP3 gene.
The FDA granted accelerated approval on the basis of two North American phase III studies, focused on safety. A placebo-controlled trial recruited over 4,000 participants to Ixchiq or placebo, and a non-placebo-controlled study recruited over 400 participants for vaccination. Commonly reported side effects included headache, fatigue, muscle pain, joint pain, fever, nausea and tenderness at the injection site.
Severe chikungunya-like adverse reactions that prevented daily activity or required medical intervention occurred in 1.6% of Ixchiq recipients. Two vaccine recipients were hospitalized with chikungunya-like adverse reactions. Some recipients had prolonged chikungunya-like adverse reactions.

The chikungunya vaccine is a live-attenuated vaccine. Chikungunya disease is transmitted by the Aedes mosquito. The chikungunya vaccine does not protect against the dengue and Zika viruses, which are transmitted by the same Aedes mosquito. (See also Overview of Immunization.)
