Oregon Vital Records has created a form that allows someone who is eligible for a vital record certificate to grant permission to order or pick up a certificate to someone who is not eligible. The form must be notarized and should accompany the order form.
To order vital records, the following information is requested. Items marked with an asterisk (*) are required.
