Originally Kaufman had two scripts one was about someone finding a portal into someone's head and the other was about someone having an affair with a co-worker. Neither was working so he combined them. For the portal part Kaufman used John Malkovich because he thought he was enigmatic and liked the repetition of his name.

The movie handles this not as a gimmick but as the opportunity for material that is somehow funny and serious, sad and satirical, weird and touching, all at once. Malkovich himself is part of the magic.

John Malkovich just seems sort of amazing. Like the story Matt Damon tells about rounders when he shows up after being hyped up by the crew for months, does a terrible first take with his corny Russian accent, then leans over to Matt and tells him he can't act.
