  5. I haven' seen the movie but remember that scene from the book. Hawaii (1966) I remember being traumatized by that when it was shown on TV in the 70s. Not the correct answer, but Ballad of Narayama (1958, Kinoshita) contains a scene of an old lady who does the same thing, she knocks her teeth out with a rock. Horrific.
