In this chapter we review the anatomy and physiology of pain pathways. We also discuss some of the physiological processes that modify the pain experience and that may contribute to the development of chronicity.

This article provides an overview of the physiological mechanisms of pain and the important pain pathways. We will discuss pain receptors, transmission of pain signals to the spinal cord and pain pathways within the spinal cord. We will also look at how pain can be modulated at different levels along the pathway.
Neural pain pathways. When the sensory neurons synapse with the motor neurons and transmission neurons in the dorsal horn of the spinal cord, pain is detected. As seen in Figure 1.1, the nerve fi bres within the dorsal horn (rear) carry information back to the spinal cord and brain.
