The diseased fat pad is characterized by inflammation, swelling, hypertrophy, and fibrosis, most often caused by trauma or prior surgery.

Acute or chronic inflammation of the Infrapatellar Fat Pad (IFP) is a common source of Anterior Knee Pain; also called Hoffa's disease, fat pad syndrome or hoffitis. Fat pad syndrome was firstly reported by Albert Hoffa in 1904 [1] .
Also known as Hoffa syndrome, fad pad impingement is exactly what it sounds like: a pad of fat gets impinged upon, or affected, in some way. This particular fat pad is located behind the kneecap tendon and in front of the connective tissue of the knee joint (synovium). It has a rich supply of nerves, so when it is damaged, you will feel it.
