Follow these steps to sign up for a MyCityofHope account. Enter your personal information. Verify your identity. Choose a username and password. If you have any questions, please contact us at 844-777-HOPE(4673). Indicates a required field. First, we need to collect some information about the patient.
If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyCityofHope help desk at 844-777-HOPE (4673) to help you regain access to your MyCityofHope account. Sign up online.

You can now view your After Visit Summary and the clinic notes that your health care provider made after your visit. You can also view any of your doctor’s visit notes that are shared with you by selecting the View Notes in MyCityofHope.
Enter your activation code as it appears on your enrollment letter or After Visit Summary®. Your code is not case sensitive. Enter your date of birth in the format shown, using 4 digits for the year.
