Both car doors were open when the police divers arrived, but Kirsty was nowhere to be found. Trevor, on the other hand, was able to flee and save his life.
After Revelations, the Hellraiser movies were not actual Hellraiser movies. The cenobite subplot is written in to keep the trademark active, the original story was not written for them. Inferno is the best example where the cenobites make no sense in context of previous movies.
