APPLICATION FOR UTILITY SERVICES (Water, Sewer, Garbage, Storm Water & Integrated Waste) The City of Fort Madison requires this form to be completed, and, if renting or purchasing on contract, a utility deposit of $100.00 must be paid before service is connected.

This includes a once-a-year cleaning of the entire system and responding to calls of sewer backups. The department also provides garbage collection once a week and recycling and yard waste pick-up twice a month for 4,000 residential units. Yard waste pick-up is available from spring to fall only. Residents must present a current City of Fort Madison water bill to receive their free recycling bin. There is a fee for any additional recycling bin needed. Recyclables can be in either in the GRRWA recycling bin or your garbage can/cart, which will need to be properly labeled.
