Early Release, Tardy, and Absent Information: A note is needed when students return from being tardy or need to be picked up early from school. This note must be turned in to the Attendance Clerk no later than 9:15 am. Please use the forms above or a hand-written note including: Student’s Full Legal Name / NO NICKNAMES. Student (s) Birthdate.
Aug 15, 2022 · Early Release Form. Use this form to obtain a waiver of the requirement that a student attends school for the full day. This is most often used for seniors to be allowed to leave school every fourth or eighth block in lieu of having a study hall.

Eden Prairie students arriving late or leaving early must have parents/guardians call the attendance line at 952-975-8001 or fill the 'Submit an Absence Form'. The message should include the parent/guardian’s name, student name, and reason for absence.
