Use the Lifeline National Verifier to enter your information, create an account, and see if you qualify.
The National Verifier (NV) is Lifeline’s centralized application system. It determines whether consumers are eligible for Lifeline. USAC manages the NV and provides customer service to consumers through the Lifeline Support Center.
You may apply directly through the Lifeline programs National Verifier online portal (lifelinesupport.org) or by mailing a paper application form (available at the National Verifier online portal after entering your state).
The Lifeline National Eligibility Verifier (National Verifier) checks consumer information when an application is submitted. This information is automatically checked against databases.
Consumers can fill out the National Verifier’s official Lifeline Application Form and Household Worksheet (if needed) and mail them to the Lifeline Support Center (address available on the application form).
