Oct 13, 2023 · Grades 2-4: Cancerous cells in the tissue look less like normal cells. Grade 5: Cancerous cells look very abnormal. Each area of prostate cancer may have a different grade, so pathologists pick the two areas that make up most of the cancer.
Prostate cancer grade (Gleason score and Grade Group) If prostate cancer is found in a biopsy sample, the pathologist will assign it a grade, which is based on how abnormal the cancer looks under the microscope. Higher-grade cancers look more abnormal, and they are more likely to grow faster.

This grading system can be used to choose appropriate treatment options. The Gleason Score ranges from 1-5 and describes how much the cancer from a biopsy looks like healthy tissue (lower score) or abnormal tissue (higher score). Most cancers score a grade of 3 or higher.

The combination of Gleason scores into a 3-tier grouping (6,7,8-10) is used most frequently for prognostic and therapeutic purposes, despite 3+4=7 vs. 4+3=7 and 8 vs. 9-10 having very different prognoses. In practice the lowest score is now assigned a 6, although it is on a scale of 2-10.
The pathologist looking at the biopsy sample will assign one Gleason grade to the most predominant pattern in your biopsy and a second Gleason grade to the second most predominant pattern. For example: 3 + 4.
