    When requesting a death certificate via the mail, you can either fill out the form and mail it in, or send a letter with specific information. NOTE: A photocopy of your government issued photo ID is required with this application.

    Applicant must present current government issued picture identification and complete a search application. The search fee for a death certificate is $ 5.00 per certificate.
