His partner of thirty years, architect Charles Gillan Jr., died of bone cancer on March 5, 2002. Winfield long battled obesity and diabetes. On March 7, 2004, he died of a heart attack at Queen of Angels – Hollywood Presbyterian Medical Center in Los Angeles. Winfield and Gillan are interred together at Forest Lawn Memorial Park in Los Angeles.
Suffering from obesity and diabetes in later life, Paul Winfield passed away from a heart attack at age 64 in 2004, and was survived by a sister, Patricia. His longtime companion of 30 years, set designer and architect Charles Gillan Jr. predeceased him by two years.
