Please follow the instructions below to request an activation code for yourself. You must be at least 12 years old to use My Sanford Chart. We need some information from you in order to grant you a My Sanford Chart account.
Sanford patients can request access online to the medical records of a child, family member or person under their care (known as “proxy” access) by visiting My Sanford Chart and selecting “Request Access to Another Person.” For your request to be processed as quickly as possible, please be sure all information, including social security numbers, is complete and correct.

OneChart Link is a web-based portal that lets you view patient data in Sanford Health's electronic medical record, One Chart.

If you have an email address on file then your My Sanford Chart Username will be sent to your email account. If you do not remember any of this information, or you do not have a valid email address on file, please contact the My Sanford Chart Technology Support Center at 1-866-808-5274 to help you regain access to your My Sanford Chart account.
If you unlink an organization, we remove all of that organization's data from My Sanford Chart. Depending on this organization and the other organization's settings, your care team might still be able to access the external data to help provide you with medical care.
