  4. You can order a certified short form or long form copy of a birth certificate for someone born in New York City in or after 1910. The NYC Health Department only accepts requests for NYC birth certificates.

  5. New York State Department of Health can provide copies of birth certificates for individuals born outside of New York City’s five boroughs. To get document, have one of these: A drivers license or non-driver photo ID. Passport. U.S. military-issued photo ID.
