It only begins to manifest when the son is old enough to comprehend the book. It's her feelings coming back to haunt her; her guilt built into a monster that is a formal reminder of her husband and an extension of her husband as a way of keeping his memory alive without any direct connotations.
While I agree that the few shots of The Babadook itself aren't particularly scary, I though the movie overall was pretty terrifying. In my opinion, it was a great example of a "less is more" approach. To me, most scenes were oozing with this underlying sense of tension and dread. The house was so bleak and depressing.
