Concentration can be held during combat, but if the caster takes damage, the concentration checks need be made to maintain the spell. Also, if the caster casts another concentration spell, concentration on pass without a trace ends.
Yes, Pass Without Trace is a horribly broken spell. So you have to compensate. Being incredibly stealthy does not mean invisible. Guards watching an brightly lit empty hallway ALWAYS see whatever is in that hallway. Further, if cast by a Ranger, 2nd level spell slots are precious, until much higher levels. (Druids, well, yeah, that is awful).
