     There are numerous exceptions where whitespace matters; this is one of them. With the space after PR, how is the preprocessor supposed to know whether (A,B) is part of the macro expansion, or its arguments? It doesn't, and simply assumes that wherever it sees PR, it should substitute ( A, B ) cout << ( A ) << ( B ) << endl ;.

     Another place where whitespace matters is in nested template arguments, e.g.:

     std::vector<std::vector<int> >

     That final space is mandatory, otherwise the compiler assumes it's the >> operator. (Although I believe this is sorted out in C++0x).

     Yet another example is:

     a + +b;

     The space in between the two + symbols is mandatory, for obvious reasons.

   • Is C# a high level language?

     From what I have read from various sources, C# is considered a 3GL language. A higher level language 4GL would look more like natural language and a 5GL language is about constraints based programming (artificial intelligence) where you define the constraints of a problem but leave it up to the computer to solve it.

     If you judge the C# language by itself (not considering the .Net framework), it is not a huge departure from other C based languages so in that sense the 3GL definition fits.

     The C programming language is now a standard, so read the C11 specification, that is n1570. Remember that a programming language is a specification (so is defined in some technical report) not a software. Ritchie's book is about an older version of C (written at a time -the 1980s- where multicore computers and multithreading did not exist or where very unusual, and Ritchie himself did not have any such computer or system). The C standard defines (loosely) the behavior of C implementations (and undefined behavior is a very important and tricky notion in it).

     There is some thread support in C11.

     You could also use C99 (or even C11) with some external libraries, such as pthreads on POSIX, providing threads.

     You could view threads (and processes, and files) as some features or abstractions provided by operating systems. A language specification usually don't tell much about them (for example, most OSes have directories, but the C standard don't tell anything about these).

     Actually, if you read the C11 standard carefully, you'll probably notice that a computer hardware is not even required (just implicitly subsumed) by it. In principle a C implementation could be a team of humans (and "running a C program on a classroom, considered as a computer" is a nice way of teaching C). In practice, using human slaves as the C implementation is unethical and inefficient, and you should prefer some computer (but what is a computer is not defined by the C11 standard).

     To show that something is Turing-complete, it is enough to show that it can be used to simulate some Turing-complete system. For example, an imperative language is Turing-complete if it has conditional branching (e.g., "if" and "goto" statements, or a "branch if zero" instruction; see one-instruction set computer) and the ability to change an arbitrary amount of memory (e.g., the ability to maintain an arbitrary number of data items).

     Then imperative languages lists C++ as such.

     Low-, high-, blah-level is all just vague terminology with no deterministic factor behind it. Traditionally, low-level languages refer to machine code and assembly, and high-level refers to everything else. More recently, we have "very high level" used for scripting languages (anything interpreted rather than compiled). I have never heard "middle" used for anything.

     That said, you will hear lots of argument on whether C or C++ are low-level or high-level languages, as some people prefer to think of C/C++ as low-level now given their relative position to other languages (25 years ago this would be unheard of). It is largely meaningless unless you agree on a definition of low-level or high-level. So you could easily come up with a definition of "middle-level" and then use that to determine whether C counts as middle-level or not.
