Definitely Signs. I saw it twice when I was a kid and it destroyed my ability to sleep. But I think the scariest moment of any movie I saw was in The Village when Ivy came face to face with the creature in the woods. When it opened its arms and started charging her, 12 year old me about passed out. It was terrifying.
Live and Let Die. There's a scene with James in a car, and as it drives off, it zooms in on a scarecrow with a camera in it's face. That thing scared the living shit out of me as a child. Forget the huge voodoo guy, or the guy with a claw instead of a hand, or the bullet that causes the guy to inflate and explode.
But onto horror films (spoilers). The first horror jumpscare I remember seeing is Jaws (1975) when the corpse of Ben Gardner appears in the boat. It terrified me as a kid but still caught me off guard when I watched it as an adult. I was also scared of the sight of the mother's corpse in Interview with the Vampire (1994).
