Even in borderline cases i don't think it matters. As a personal anecdote my mom interviews people (non medicine) and she never considers thank you letters.
Thank you emails are not important. If you want to send one, the average student sends a thank you of 115 words, 14 hours after their interview. They mention a specific point of conversation, sign off with some form of sincerely or best, and do not typically leave their AMCAS ID. Again, none of this matters.

You should always send one, but I wouldn't expect a response. It doesn't mean that they don't like you, but it's usually not typical to send a reply to a thank you note. I should add that I'm from the US. From what I've read, it's not customary to send a thank you after an interview in most European countries.
