The ASQ-3 includes 21 questionnaires, one for each of the following ages: 2, 3, 6, 8, 9, 10, 12, 14, 16, 18, 20, 22, 24, 27, 30, 33, 36, 42, 48, 54, and 60 months. The questionnaires are designed to be completed by parents or other primary caregivers in less than 15 min and scored by a professional in less than 3 min.
