Despite its name, Rocky Mountain spotted fever (RMSF) occurs in practically all of the United States and throughout Central and South America. Small-vessel vasculitis can cause serious illness affecting the central nervous system, lungs, heart, kidneys, liver, and spleen; untreated mortality is about 20%.
Rocky Mountain spotted fever is a tickborne disease first recognized in 1896 in the Snake River Valley of Idaho. It was originally called “black measles” because of the look of its rash in the late stages of the illness, when the skin turns black. It was a dreaded, often fatal disease, affecting hundreds of people in Idaho.

Rocky Mountain spotted fever is probably the most common rickettsial infection in the United States. It was first recognized in the Rocky Mountain states but occurs throughout most of the continental United States. It is most common in the southeastern and south central United States (North Carolina, Oklahoma, Arkansas, Tennessee, and Missouri).
