Diagnosis for Lyme disease must be made by a healthcare provider experienced in recognizing Lyme disease. Diagnosis is usually based on symptoms and a history of a tick bite. Testing is generally done to confirm the diagnosis and rule out other conditions.
Nymphal ticks cause most cases of Lyme disease. Because nymphs are as small as poppy seeds and their bite is painless, people often don’t realize they have been bitten. Adult ticks can also infect humans, but are easier to spot and remove. Not all ticks are infected.
Serologic tests are highly sensitive in patients with disseminated Lyme disease, and diagnosis relies on signs and symptoms supported by results of testing. Two-step serologic testing is recommended using validated first- and second-tier tests according to a standard or modified two-test algorithm.
