The Division of Vital Records offers two types of birth certificates available for legal purposes. Please indicate your choice below and return this form with the proper fee and a legible copy of your non-expired, government issued photo ID. Illinois Law (ILCS410/535/25(1)) requires advanced payment for the search of birth record files.
Our hours are Monday-Friday 8:30am - 4:30pm. You need to have your ID and payment. We accept cash, check, or credit card. 2. Fees for Birth Certificates are $16 for the first copy and $6 for each additional copy of the same record. A total of 3 copies of the same record may be ordered at one time. If additional copies are needed, please contact the County Clerk's Office at 630-232-5950.
