Health Officer Order: Health Care Facility Transfers. To reduce the spread of infections, including those caused by multidrug-resistant organisms, the Sacramento County Public Health Officer has issued an Order requiring health care facilities that transfer a patient to another facility to submit a completed Health Care Facility Transfer Form to the receiving facility.
