Unfortunately, ILDs are not biopsied often enough for pathologists to gain first hand experience in view of the exceptionally broad spectrum of this pathology. To circumvent this problem, I teach pathologists to use six basic histopathological patterns in the evaluation of the ILD biopsy specimen.

Interpretation of lung biopsies in a patient with ILD is best accomplished using a multidisciplinary approach that results in a composite clinico-radio-logic-pathologic diagnosis. Unfortunately, this is not always realistic in many clinical practice settings.
