ABOUT LIFE. “Living Independence for the Elderly,” or LIFE, is a Pennsylvania program that provides comprehensive health and support services to seniors who wish to continue living independently in the community.
LIFE is a Medicaid and Medicare program that serves older Pennsylvanians in their homes and communities. LIFE has local providers that pay for, manage, coordinate, and provide all physical and behavioral health services. Care is managed and delivered by a team including a doctor, nurse, social worker, therapists, and other care staf.
Living Independence for the Elderly. LIFE programs give seniors the support they need to live in their own home or with family. Each participant receives full circle medical and social care based on an individual care plan. LIFE services are free to seniors who qualify. InnovAge LIFE, known nationally as the Program of All-inclusive Care for the Elderly (PACE), can help by providing healthcare and social services to seniors in Philadelphia - without having them move to a nursing facility. InnovAge operates five LIFE centers for older adults in Philadelphia.
