The Mad Mary thread is a relic from when you'd show up in the villiage and head right to the castle. It's inclusion in Curse of Strahd is a huge mistake. Or at least, it's inclusion at that point. It should occur if/when the players return to the Village of Barovia before going to the castle.
In my game, players heard the plea of Mad Mary, and 12 sessions later got to go to a dinner at Ravenloft. Gertrude was there, polite and smiling, along others from the castle. She had been turned by then, and asked the players if they could escort her back to Barovia.
