Incantation was formed in New Jersey, USA by John McEntee and Paul Ledney, two musicians who'd previously played together in Revenant. In the mid-'90s, McEntee moved the band to Cleveland, Ohio. He then, in the late-'90s, moved it again to Johnstown, Pennsylvania.
