Mar 26, 2021 · The standard serologic test for diagnosis of RMSF is the indirect immunofluorescence antibody (IFA) assay for immunoglobulin G (IgG) using R. rickettsii antigen. IgG IFA assays should be performed on paired acute and convalescent serum samples collected 2–4 weeks apart to demonstrate evidence of a fourfold seroconversion.
Serology. Gold standard is IFA for IgG antibodies in paired serum samples. First sample should be obtained in the first week and will be low or negative. Second sample should be obtained 2-4 weeks later and should show a four fold increase in IgG if positive. Both IgG and IgM begin to rise at the end of the first week of symptoms. Limitations.
