It’s easy to find a provider with our Find a Provider tool. You can search by the type of plan or coverage. You can search by the specialty of the provider or facility. You can search by the area of interest. You can also search by last name, gender, and hospital affiliations.

Prepare to give HHSC the following information: Your child’s social security number. Your family income within the last 30 days. (EX: check stubs, most recent income tax, W-2 form, wage forms and/or a letter from your employer.) U.S. Birth Certificate or proof of legal residency status for your child.
Texas Children’s Health Plan offers no-cost health coverage for children and young adults under the age of 21 with disabilities who cannot afford health insurance. The plan helps families manage their medical needs and acts as a care coordinator between physicians and families.
