Valentino is a bisexual porn star whose girlfriend Mary Carmen is a fiery waitress. Their relationship hits a problem when Valentino falls for Gary, a cute Midwesterner. Carmen isn't about to give up, and soon she and Gary are fighting over Valentino. But when the porn star contracts AIDS and begins.
Gary of the title (Thomas Jane) is an inarticulate bumpkin from the Midwest whose new life in New York has reduced him to selling his blood. He meets Valentino (Vincent D'Onofrio), a semi-famous, now apparently retired, bisexual porn actor, who molests him on the street after having known Gary for maybe two minutes.
