Would not worry about counterfeits. 500,000 and 200,000 and a few 100,000 would be fine. Hotels and big restaurants will have change. You will pick up plenty of change as you move around.
Do not change it for USD to change in Vietnam. Lose all the way doing that. If you're from somewhere exotic Venezuela, Iceland or wherever, your local currency is probably going to be difficult to change. So, maybe having some USD stashed away for emergencies makes sense. Also don't buy VND outside of Vietnam.
