The manga doesn't leap far into the future, so there is no marriage or children at the end. It ends with Anzai meeting Tsukasa's parents and them eventually approving of him. They do talk about their future and agree to marry but Anzai says he still needs some more time to grow.
The end of the series is really the beginning. The open door doesn't represent Kou being 100% fixed and jumping into a happy relationship. The open door just means that Futaba and Kou's friends can enter his life and help share his pain, the same way that he often shared theirs throughout the series.

This last episode was great and managed to actually conclude the storyline somehow. I can only assume the manga concluded an arc and then continued after a timeskip, so 12-ep anime could have definitive conclusion (I thought it would be "go read the manga" based on the amount of characters and plotlines alone).
