Apr 18, 2024 · Henry Ford MyChart offers a secure channel to conduct scheduled video visits with your physician, or an on-demand video visit with an on-call physician, through the patient’s personal smartphone or mobile device.
Login to MyChart. From the menu button find Sharing Hub. From here, you can easily download and share information with others. You can also request a release of your medical information to other doctors, insurance companies, and more. The “Share Everywhere” option allows a non-Henry Ford doctor one-time, limited access to your record.
If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyChart help desk at 313-876-7951 and select MyChart account from the menu, to help you regain access to your MyChart account.
Create a log in account at henryford.com/mychart. Download the free MyChart app from the Apple App Store or Google Play. Log in to MyChart with the account you created on the app or at mychart.hfhs.org. Update your communication preferences.
Choose a username and password. If you have any questions, please contact us at 313-876-7951. This form should be completed to request a Henry Ford MyChart account for yourself. You must be at least 18 years old to complete online verification.
Access your medical records online. Henry Ford Health MyChart allows you to review portions of your medical/surgical history including lab results, test results, current immunizations, provider notes. It is free to use, completely secure, and available 24 hours a day.
Enter your activation code as it appears on your enrollment letter or After Visit Summary®. Your code is not case sensitive. Enter your date of birth in the format shown, using 4 digits for the year. Please enter the phone number through which we can best reach you.
