The Department of Health issues certified copies of vital records (or “certificates”) for births, deaths, stillbirths, fetal deaths, marriages, and divorces that took place in the state of Washington. We can help you with: Ordering vital records (certificates) Birth record. Death record. Stillbirth or fetal death record. Marriage or divorce record.
To order a birth certificate, noncertified informational copy of a birth record, and Heirloom birth certificate, you must provide: First, middle, and last name of the subject of the record. Date of birth (Month, Day, Year).
