When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. If you have any questions, please contact us at 404-727-8820. Follow these steps to sign up for a MyChart account. Enter your personal information.
Enter your activation code as it appears on your enrollment letter or After Visit Summary®. Your code is not case sensitive. Please enter the last 4 digits of your Social Security number. Enter your date of birth in the format shown, using 4 digits for the year.
If you do not remember any of this information, or you do not have a valid email address on file, you will have to contact your MyChart help desk at 404-727-8820 to help you regain access to your MyChart account.

Request Patient Portal Access. Emory Healthcare uses different electronic medical records to provide patient care. Because of the way our technology works, this means we also have multiple patient portals – the Blue, Gold, and Eye Center portals.
Laboratory results are available to patients through the Emory Patient Portal. Please note that although most laboratory results are posted to the secure Patient Portal, more sensitive test results along with Anatomical Pathology and genetic results will not be available through the Portal.
