Cassidy ran for the U.S. Senate in the 2014 election, in which he was endorsed by Republican Senator David Vitter. He defeated three-term incumbent Democratic Senator Mary Landrieu in the runoff election held on Saturday, December 6, 2014, receiving 56% of the vote to Landrieu's 44%.
In 2023, he became Ranking Member of the Senate HELP Committee. He is the first physician to sit as HELP Ranking Member or Chairman since 1933, when it was called the Education and Labor Committee. Bill is married to Dr. Laura Cassidy, and they have three children and one grandchild.
