MyChart is for Children’s Health patients and their caregivers. To have a Children’s Health MyChart account you must be either a patient of Children’s Health (aged 14 or older), or the parent or legal guardian of a patient (any age). Children in foster care and their caregivers are not eligible for MyChart accounts.
That person will need to accept the invitation and confirm that they know your date of birth. Once access is granted, access to your medical record will appear in the proxy section of your proxy's MyChart account. When you are in your child's record in MyChart, go to My Record > Health Summary and select the Immunizations tab. You can see your child’s immunizations and when they were given. To print from the mobile app, click the three dots at the top of the page. To print from the web site, click the printer icon.
