Mar 4, 2011 · National Lifeline Eligibility Verifier Public Notice : WCB announces that the National Lifeline Eligibility Verifier will soft launch on October 11, 2019 in Alabama, Arkansas, Louisiana, Maryland, Massachusetts, New Jersey, Oklahoma, Puerto Rico, South Carolina, and Washington.
The Lifeline National Eligibility Verifier (National Verifier) checks consumer information when an application is submitted. This information is automatically checked against databases.
Visit the National Verifier. Sign in using the applicable National Lifeline Accountability Database (NLAD) or National Verifier-only credentials. Use an interview-style approach to ask the consumer for the required information, complete the application, and upload documentation (if needed).

Aug 14, 2019 · To locate a Lifeline provider, go to www.lifelinesupport.org, or call USAC's toll-free number (1-888-641-8722). Lifeline applicants will be required to provide certain information to the provider, state agency or through the federal National Eligibility Verifier, when active (depending how subscribers in their state sign up for Lifeline).
